Person Centred Planning

The aim of Person Centred Planning is to focus on the individuals rights and promote independence. It is a flexible ongoing process assessing what is needed to make effective changes to reach their dreams and ambitions.

It usually involves family, friends, neighbours, support workers and other professionals involved in their lives.

The outcome is for the individual to identify specific actions allowing them to achieve their goals and identify their fears and help them access the tools to overcome them. This approach encourages the individual to become more involved in taking control for achieving their goals.

There are different planning styles in the way information is gathered and whether the emphasis is on day to day targets or a longer term objective.

Landau uses a variety of tools to develop individual action plans covering initial and long term objectives.
